Pop - Up Class Registration

This Class is being filmed as a part of a project documenting the journey and recovery of Mataya Dade, a triple-negative breast cancer survivor, and close friend of Renee Baldwin.

NOTICE OF FILMING AND PHOTOGRAPHY

By signing up for this class, you acknowledge that the session will be filmed as part of a documentary for the podcast You Survived! Now What?. By participating, you agree to the following:

  1. You may be photographed, filmed, and recorded during the session.

  2. Your likeness, voice, and participation may be used in any media, now known or later developed, in perpetuity, for promotional, commercial, and creative purposes related to the podcast and documentary.

  3. You will not receive compensation or credit for your appearance.

If you do not wish to appear in the footage, please refrain from attending. Thank you for your understanding and cooperation.
